Position:
Admissions Nurse Manager Edenbrook Appleton, a skilled nursing facility in the Eden Senior Care Network, is now hiring an Admissions Nurse Manager for our fantastic facility! As the Admissions Nurse, your primary responsibility is to represent Edenbrook's s services to our residents and their loved ones in a way that results in an admission appropriate to our resident's needs.
As an Admissions Nurse Manager, you'd be part of the Nurse Management team and take part in the on-call rotation.
Responsibilities:
Obtain agreement for admission of referred patients and families who meet admission criteria.
Build effective referral source relationships to support the high quality and responsiveness expectations associated with Eden Senior Care.
Maintain clinical and professional competence.
Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient's family has concerning hospice and palliative care.
JOB REQUIREMENTS Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas.
Ability to communicate effectively with patients and families, physicians, referral sources and colleagues Must be willing to work every 4th weekend on call rotation.
